LVN / LPN - LTC/SNF
Location: Ludington, MI
Schedule / Job Type: Full-time, 5 shifts per week, Mids shift (3pm-11pm), with options for 8-hour or 12-hour shifts, including block scheduling.
Key Responsibilities:

• Administer medications and monitor patient conditions.
• Conduct head-to-toe assessments.
• Assist with patient care in a Skilled Nursing Facility setting.
• Support charge nurse duties as needed.
• Collaborate with interdisciplinary teams, including PT/OT, dietitians, and social services.
• Provide quality care for patients with diagnoses such as CHF, COPD, DM, dementia, and rehab needs.

Required Qualifications:

• LPN license in good standing.
• Basic Life Support (BLS) certification.
• At least 1 year of LTC experience.
• Comfortable with IV administration and antibiotics.
• Ability to work in a fast-paced long-term care environment, including first-time traveler considerations.

Preferred Experience / Skills:

• Experience working in skilled nursing facilities or SNFs.
• Flexibility to float throughout the facility.
• Comfortable working with dementia and complex patient needs.
