- Full-time, days, 12-hour shifts, 3 days per week
Key Responsibilities
- Develop and implement personalized care plans in collaboration with physicians, patients, and families.
- Conduct ongoing assessments of patients’ clinical, functional, and emotional status to monitor progress and identify issues.
- Coordinate patient care through interdisciplinary team communication and case conferences.
- Educate patients and caregivers on self-care techniques, medications, diets, and health practices.
- Document evaluations, treatment goals, and patient progress accurately and timely.
- Communicate significant changes in patient condition to physicians and team members, ensuring seamless care.
Required Qualifications
- Current RN license in Vermont or New Hampshire, or unencumbered Compact License.
- Graduate of an accredited nursing program.
- Minimum of 2 years’ experience in home health care.
Preferred Experience / Skills
- Experience working within interdisciplinary teams.
- Strong communication and patient education skills.
- Familiarity with home health care regulations and documentation.
