Key Responsibilities
- Collaborate with a consortium-wide team to support inpatient Care Coordination and Utilization Management.
- Coordinate the local Critical Access Hospital (CAH) Swing Bed/Transitional Care program.
- Lead interdisciplinary team meetings for inpatient care.
- Review all admissions to develop personalized, clinically appropriate treatment plans aligned with patient needs, preferences, and resources, ensuring timely and appropriate discharges.
- Work with providers, leadership, and support teams to ensure treatment plans meet regulatory and payor criteria.
- Document care and utilization management activities accurately and communicate effectively with payors.
- Educate patients and families regarding healthcare benefits and treatment plans.
- Assess patients to gather psycho-social information, provide interventions, and refer to professional support services when needed.
- Develop and communicate safe, coordinated discharge plans with the care team and families.
- Support peer review processes as part of the healthcare consortium.
Required Qualifications
- Current, unrestricted Registered Nurse license in Alaska or another U.S. state.
- Bachelor’s degree preferred; must have applied for an Alaska nursing license prior to start date.
- Active Alaska nursing license for agency staff.
- Case Management certification from a recognized organization within 24 months of hire.
- BLS certification.
- ACLS and PALS certifications preferred.
- High school diploma or equivalent required.
- Clinical competency required within 3 months of hire and every three years.
- High-risk competency required within 3 months of hire and annually.
Preferred Experience / Skills
- Minimum of 5 years clinical nursing experience.
- At least 1 year of experience with chart review, risk management, or related quality services.
- Knowledge of medical record review, medical necessity guidelines, and regulatory compliance.
- Strong verbal and written communication skills.
- Excellent organizational abilities.
- Ability to work collaboratively within an interdisciplinary team.
- Ability to build relationships with colleagues, community partners, and patients.
Physical Demands
- Regularly speak and hear.
- Frequently stand, walk, sit, use hands, and reach.
- Occasionally climb, balance, stoop, kneel, crouch, or crawl.
- Ability to lift and move up to 50 lbs.
