Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, and risk factors in accordance with RN scope of practice and applicable policies.
• Develop, implement, and evaluate individualized plans of care in coordination with patients, families, and the inter-professional team using all phases of the nursing process.
• Administer and monitor patient care therapies and procedures, recognizing and communicating changes in patient condition in a timely and appropriate manner.
• Facilitate admission, transfer, and discharge activities, including discharge teaching and coordination with case management and other team members.
• Delegate tasks appropriately and oversee team members to ensure accuracy and completeness of delegated patient care activities.
• Participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky.
• Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN).
• Current Basic Life Support (BLS) certification.
• Effective verbal and written communication skills with the ability to collaborate across care team disciplines.
• Ability to assess age-specific patient safety needs and implement appropriate safety measures.
• Proficiency with or ability to be trained on relevant computer systems and clinical applications.

Preferred Experience / Skills

• Previous clinical nursing experience in a medical surgical or telemetry setting.
• Bachelor of Science in Nursing (BSN).
