Key Responsibilities
- Evaluate and treat patients requiring physical therapy services in a skilled nursing or long-term care setting
- Develop and implement individualized treatment plans based on patient needs and goals
- Monitor and document patient progress, adjusting treatment plans as appropriate
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
- Active Physical Therapist (PT) license in the state of Nebraska or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Ability to commit to a guaranteed weekly schedule of 36–40 hours depending on shift length
Preferred Experience / Skills
- Prior experience in a skilled nursing facility (SNF) or long-term care environment
- Familiarity with geriatric patient populations and related rehabilitation needs
