Key Responsibilities

• Evaluate and treat patients requiring physical therapy services in a skilled nursing or long-term care setting
• Develop and implement individualized treatment plans based on patient needs and goals
• Monitor and document patient progress, adjusting treatment plans as appropriate
• Collaborate with interdisciplinary care teams to ensure comprehensive patient care
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Nebraska or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Ability to commit to a guaranteed weekly schedule of 36–40 hours depending on shift length

Preferred Experience / Skills

• Prior experience in a skilled nursing facility (SNF) or long-term care environment
• Familiarity with geriatric patient populations and related rehabilitation needs
