Contract | Nights | Onsite | 8-hour and 12-hour shifts available | Every other weekend and holiday required

Key Responsibilities

• Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
• Manage and monitor patients with diagnoses including dementia, diabetes, hypertension, chronic kidney disease, and fall risk
• Assist residents with activities of daily living (ADLs) as needed
• Administer medications and treatments in accordance with physician orders and facility protocols
• Document patient care accurately using the facility's electronic medical record system (PCC)
• Maintain resident safety and comfort across assigned unit with an average ratio of 1:22

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of Nebraska or compact equivalent
• IV Certification (IV-C) required
• Experience in a long-term care or skilled nursing facility (LTC/SNF) setting
• Proficiency or ability to learn PCC (PointClickCare) EMR system
• Ability to work every other weekend and every other holiday as required

Preferred Experience / Skills

• Prior experience caring for patients with dementia, chronic kidney disease, or fall-risk populations
• Familiarity with ADL assistance protocols in a skilled nursing environment
• Experience with both 8-hour and 12-hour shift schedules
