Key Responsibilities

• Provide direct patient care in a long-term care and skilled nursing facility setting
• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor and document patient conditions, reporting changes to the supervising RN or physician
• Assist with patient assessments and contribute to individualized care planning
• Collaborate with interdisciplinary care teams to ensure quality resident outcomes

Required Qualifications

• Active and unrestricted LPN or LVN license in the state of Nebraska
• Experience in a long-term care or skilled nursing facility environment
• Ability to work day shift hours within a Sunday through Saturday work week
• Willingness to commit to the full contract duration with a minimum 14-day notice required for any schedule changes or early termination

Preferred Experience / Skills

• Prior experience in a LTC or SNF setting with a stable work history
• Familiarity with electronic health records and clinical documentation systems
