Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient or clinic-based setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver hands-on therapeutic interventions in alignment with plan of care goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with the care team to ensure continuity and quality of patient care

Required Qualifications

• Active Physical Therapist license in the state of Arizona (permanent license required)
• Prior experience in a physical therapy clinical setting
• Ability to meet credentialing and contract quality assurance requirements
• Must provide proof of license application within 5 business days of booking if licensure is pending

Preferred Experience / Skills

• Experience working in an outpatient or specialty physical therapy environment
• Familiarity with documentation systems used in high-volume clinical settings
