Key Responsibilities

• Provide physical therapy services to patients in accordance with prescribed treatment plans.
• Complete documentation and required paperwork directly with the facility prior to start, outside of standard clearance processes.
• Ensure compliance with licensing and credentialing requirements for the assignment.
• Set up and operate necessary equipment during therapy sessions.
• Maintain accurate and timely patient records and progress notes.
• Adhere to facility policies regarding dress code, parking, and professional conduct.

Required Qualifications

• Active Physical Therapy license in the state of Alabama.
• Experience in setting up and delivering physical therapy treatments.
• Ability to complete required documentation accurately and efficiently.
• Credentialing requirements met for contract engagement.

Preferred Experience / Skills

• Prior experience working in outpatient or clinical settings.
• Familiarity with facility-specific equipment and protocols.
• Excellent communication and organizational skills.
