Key Responsibilities
• Provide skilled nursing care to adult patients in a home health setting
• Conduct patient assessments, develop care plans, and document clinical services
• Visit up to six patients per day, ensuring quality care and compliance with protocols
• Communicate effectively with patients and healthcare teams to coordinate care
• Utilize HomeCare HomeBase and Oasis for charting and documentation
Required Qualifications
• Minimum of 1 year recent skilled home health experience within the last 3 years
• Active WA license or compact license
• Current BCLS certification
• Own reliable transportation for local travel (up to 50+ miles radius)
Preferred Experience / Skills
• Familiarity with home health documentation tools (HomeCare HomeBase/Oasis)
• Ability to work independently with strong time management skills
