Schedule / Job Type
- Full-time, Days shifts, 5 days per week, 8-hour shifts
Key Responsibilities
- Provide skilled home health nursing care to patients in their homes
- Conduct wound care and monitor vital signs
- Educate patients on medications and treatments
- Communicate patient status changes to supervising RN or MD
- Manage pain assessments and interventions
- Handle pharmacy calls and medication refill requests
Required Qualifications
- At least 1 year of experience in skilled home health care, including wound care and patient teaching
- Valid BLS certification
- Knowledge of infection control protocols
- Strong communication skills for updating clinical teams
Preferred Experience / Skills
- Prior experience with patient education and medication management in a home health setting
